Access to your hospital records

Your Medical Records

You can access a portion of your medical records online any time through Patient Portal. Your patient portal account includes test results and other details about your medical care.

At Anna Jaques Hospital, the Health Information Management Department maintains patient medical records. You have the right to review or get copies of your medical records for a fee.

How To Request Copies of Medical Records

Call Health Information Management at 978-463-1155 and press option 1 to request copies of your patient medical records. The office is located at 25 Highland Ave. We are open Monday through Friday from 8:30 am to 4:00 pm (except holidays).

If you need a paper copy of your medical records, you must complete and sign an authorization form. This form also allows you to send your medical information to another health care provider. There is no charge for records delivered directly to a medical office.

To request copies of a medical record, you must be the patient, or be the patient's guardian or legal representative.

How To Request Images

Getting your diagnostic images to share with another doctor is easy.

Your images will be added to your electronic health record after our imaging specialist reviews them. If you would like a copy of one of your images, please contact our image archive staff at 978-463-1120.

You’ll need to complete and sign our radiology authorization form.

What To Expect

If you are requesting your medical records for continued care, please let us know. We can provide an abstract of your records at no cost to you.

You can expect to receive your medical records in 10 to 21 days. How long it takes depends on the number of records requested and the date of treatment. We mail all records to the address listed on the authorization form.

To protect patient privacy:

  • We do not fax medical records, except for emergency care.
  • We do not email patient information.

Health Information Management does not provide radiology films or copies of hospital bills.
