Improving Quality of Care and Keeping Patients Safe
Anna Jaques Hospital is committed to delivering quality healthcare services in the safest environment possible. In recognition of that commitment, we work diligently to create positive patient care experiences and reduce hospital readmissions.
In 2015, Anna Jaques Hospital received funding from the Health Policy Commission in the form of a “CHART” grant with the primary goal being to reduce the rate of 30-day inpatient readmissions among our highest utilizing patients by 20% and reduce the rate of returns to the Emergency Department among the same patient population by 25%.
More than 700 unique patients who use our hospital frequently (high utilizers) were reached through the grant over a two-year period. The target population ranged in ages from 18 to over 90 and included patients with a wide range of diseases, chronic medical conditions and behavioral health complications, as well as those with limited or no practical access to primary care.
Other efforts to ensure safe, high-quality healthcare for both patients and their families include:
- Improving testing efficiency for faster outcomes and better treatment results.
- Limiting the spread of disease while inside the hospital through advanced infection surveillance, prevention and control processes.
- Maintaining engagement with inpatients after discharge to ensure we continue to meet the highest quality standards.
- Fine-tuning our methods to avoid preventable harm and prevent long-term hospitalizations.
ICU Staffing Ratios
The ICU/CCU staff cares for critically ill adult medical/surgical patients that require intensive, advanced care. Continuous observation and intervention are provided to prevent complications and restore health. Expert staff are able to apply technical expertise while providing emotional support for patients and families.
